The below terms and conditions apply to situations in which the customer makes a material change to its business during the term of the Agreement. Please note that the below terms and pricing are subject to adjustment at any time. The scenarios listed below are not intended to be comprehensive and Customer understands and agrees that Nextep may charge fees to similar scenarios (not explicitly stated herein) in its sole discretion.
Customer agrees to notify Nextep as soon as reasonably practicable of any material business change in a manner to be determined by Nextep and agrees that Nextep shall have forty-five (45) days from the date Customer notifies Nextep of a material change in its business structure to implement and update its records and systems of the change.

Customer must execute a new Agreement with the new business entity. |
| New Ownership of Customer’s Business* | $20 per WSE one time set-up fee with a $1,000 minimum | Customer must complete Nextep questionnaire on the details of the new entity including, but not limited to: changes in WSE info, benefits, operations. Customer must execute a new Agreement with the new business entity. |
*Please note that these business changes may be subject to underwriting for Nextep’s benefit and workers’ compensation offerings.
